During your Providence Health & Services enrollment, you have the opportunity to apply for additional voluntary coverage.

This is an individual policy that pays a percentage of your monthly salary if you are unable to work for several weeks or months due to a covered injury or illness.

Newly eligible caregivers – You will be sent a postcard when you are eligible to enroll. Contact the Benefits Service Center for PH&S caregivers at (888) 615-6481 to speak to a benefit representative who can answer any questions. During this initial eligibility period you are able to apply for coverage without answering any health questions or taking a medical exam. When you're ready, access the enrollment system at www.hrforcaregivers.org to make your election. 

All other caregivers – If you do not enroll on this plan when you were initially eligible you can enroll during future annual enrollment periods. You will be required to answer a few health questions, if you enroll at a later date.
